Is there a way to state that you don't want to be ressucitated ? I've read about the US surgeon who had a "do not ressucitate" tattoo on his chest, but it's rather long and doesn't travel too well.
Maybe there could be an international logo tattoo for people who want to be left alone if they collapse.
