I've never tried applying traction without painkiller but I had it applied to me (I broke my femur when I was 11 - has to have it re-set 3 times, only got painkiller in the ambulance - when they re-did it in hosptial they did it without. What happens is a nurse gets to hold you down while the 2 fattest doctors pull on your leg & your mum faints in the corner. Oh, and it hurts. Quite a lot.
