As far as I understand it, it for for the EMT at the scene to decide whether someone is 'mentally competant' or not. Someone exiting from a diabetic episode is often unfit to decide on whether they need further treatment.
Tom is a professional who saves lives regularly. He decided the man needed to go to hospital for further treatment. If someone of his experience says you need to go to hospital then you really need to go, despite whatever wishes you may be expressing in your half lucid state.
Tom did the right thing, he saved the mans life. However you play it that is more important than the wishes of a patient who has just come out of a hypo.
