Just like you shouldn't give up on a clearly incapacitated patient, you also shouldn't force clearly minded individuals to undergo procedures they don't want. Personally, I think Tom walked a slippery slope, but managed to stay on track. He has realised that this is a tricky situation; one that requires care and thought.
