...and also, did you not spot the potential error *before* any such database has been put in use. All the radiographers that I knew had enquiring minds when it came to the patient that was about to be irradiated - checking that it was the correct foot to x-ray and the like. No computer system will change that - try booking in a foreign gentleman with half a dozen ways to spell his name and you'll see how easy it is for a computer system to get horribly mucked up.
