Where I work, that patient would be taken to a 24 hour medical clinic. I explain to them that what they are feeling isn't really an emergency, and that it is inappropriate to take them to the ED. However, if they wish to see a doctor I am happy to take them to the clinic or, in business hours, their GP. Of course, they will have to pay the clinic (as well as paying for the ambulance). I then give them the option to reconsider whether they really need to see someone. If they reconsider, they're sane. If not, well, they probably do need help...
