I have no idea how your council deals with such things, or even if they mean the same thing as we do - essentially for us it is so we can alert social services that we have immediate concerns for the person's safety - for example they live in an unheated house and it is winter, or their mobility is such that they are likely to keep falling over.
I would imagine that some time this year a social worker will come and talk to you - beyond that I have no idea I'm afraid.
