A lot of medical textbooks already have the scratch-off panel approach. The codes are all unique (the scratch-off panel is actually on a sticker stuck inside the front cover), so can't be shared.
You register the book against an email address and password - and people aren't keen on sharing those because if they're caught they themselves also lose access.
